What does Injured and Healthy mean in Diablo 4?

Diablo 4 seems to focus a lot on specific states when it comes to its stats. Many gear pieces and Aspects provide additional bonuses when attacking enemies other under the effect of these states. Injured and Healthy are two of many of the states, but it isn't exactly clear what they mean. Assumptions can be made, based on their names, but I assure you they are incorrect. Instead, allow me to provide the insight needed to understand what Injured and Healthy exactly mean in Diablo 4.

Diablo 4 - Healthy and Injured, Explained

The Healthy and Injured states work on total health thresholds to determine whether or not additional effects take hold against affected targets. For clarity, here's a breakdown of both before we jump to into why they matter in the grand scheme of things:

  • Healthy - If a target is above 80% of their maximum health, they are considered in the Healthy state.
  • Injured - If a target is under 35% of their maximum health, they are considered in the Injured state.

Note that both these states affect you exactly the same way they affect monsters. These two states are important because some Aspects and gear provide significant power boosts against targets in either one of these states. As a Rogue, I use an Aspect that gives me a substantial Critical Strike chance boost while my targets are in the Injured state. I'm talking around 35% increased crit rate here, basically making the Injured state an execute phase for me. This matters especially against high-health encounters, like bosses, at the endgame. Note that this is a stacking multiplier like everything else in Diablo 4, thus these two statuses are equally as important as the others.

If an enemy is not above Healthy or Injured, lower than 80% health but higher than 35%, then bonuses that affect these two states do not work. In fact, there are no additional effects for enemies between the lines, so you'll have to rely on traditional damage until one of these two states is activated. One final note, there is no visual indicator of these states taking effect. You just need to pay attention to your enemy's health bar.
