500-year old Mera Wine is one of the cooking items you need to 100 percent your Adventurer's Tome for Rethramis in Lost Ark. If you hover over it, it does give you a slight hint on the location of where to find this collectible.
The hint mentions the second dungeon in the game, Aquilok's Head, in the Ankumo Mountain area. You need to find a bonfire next to a pile of junk in this place. Inside the dungeon, this collectible is entirely missable, as it's in a hidden area. Since the dungeon has shortened in size compared to the Closed Beta, the wine is in a new location entirely.
The hidden entrance to this location is not far into the dungeon, but keep a lookout for a lever you can push on the left side of the screen.
This will cause the boulders behind it to fall, and you can jump down off the cliff into a hidden area not shown on the map. Follow the bottom wall of this location and you'll soon come across a satchel to investigate.

But there is a slight catch. You don't actually acquire 500-year-old Mera wine—you get 499-year-old Mera wine. Don't worry. It's entirely intentional. If you hover over the item in your inventory, it will say duration two hours. You need to wait two hours in real-time, and the 499-year-old Mera wine will turn into 500-year-old Mera wine.
If you use the 499-year-old Mera wine, nothing will happen. You'll get a message pop-up saying:
Ah, I feel like throwing myself into the river. I really do. A 500-year-old-Mera Snake Wine just slipped through my fingers.
Why, you idiot, why couldn't you wait? Why? I don't deserve to call myself a gourmand anymore.

If you jump down the cliff inside the dungeon, you have gone too far and can't get go back to retrieve the collectible. You have to repeat the dungeon to acquire it.
