How to change worlds in Once Human
First, to answer the titular question, yes, you can change worlds in Once Human. In fact, there are two ways: via teleportation and server hopping. Both can be done relatively easily, though one requires a bit of a sacrifice if you've already started playing.

How to change worlds via Teleportation
You can use Teleportation to travel to warp to a different, less crowded server. This is done by heading to a Teleportation Tower.
Select the "Change to another world" option, and you'll be shown a list of worlds you can choose from. These worlds also display their relative populations, which means you decide if you want to deal with more people or less, at the click of a button.
In most cases, you'll want to change worlds if the one you're already on gets too overcrowded. After all, there is a lot of stuff to build in Once Human, and more stuff needs more space. Can't have a neighbor be too close to your property, after all.
How to change worlds via Server List
Alternatively, though not practical, you can also choose a different server from the initial one you selected from the Server List screen when you enter the game. This is helpful if you want to change to play a new character on a different server or if your current main server is full but you want to play anyhow. Just remember that characters are per server, so you cannot transfer different characters onto different servers.
