The only way to unlock the Ninja Vest perk is by completing five daily challenges in Modern Warfare 3. You can find the daily challenges in the MW3 main menu under the Armory Unlock system.

However, you must activate the Ninja Vest perk before attempting the daily challenges. Once you complete the primary objective, the new perk is all yours, and you can equip it in your loadouts from the 'Customize' section.
