How to complete Millicent’s Questline in Elden Ring

Time to die, a lot.

There's plenty of NPC quests to find and finish in Elden Ring, but some of them are tricky to keep track of, especially when NPCs like Millicent move around often. By completing Millicent's questline, you also unlock the key item to remove Frenzied Flame if you accidentally stumbled upon the Three Fingers, so here's how to complete her questline.

How to start Millicent's questline in Elden Ring

Millicent is located in Caelid, next to the Aeonia Swamp. If you happen to enter the swamp, you'll be invaded by Millicent, but it does not alter anything about her quest.

You first need to speak to Gowry in his shack, south of Sellia, Town of Sorcery. After speaking to him, he wants you to help a girl called Millicent, who has Scarlet Rot. Gowry wants you to find the Unalloyed Gold Needle located in the Swamp of Aeonia.

Where to find the Unalloyed Gold Needle in the Swamp of Aeonia

The Unalloyed Gold Needle can only be obtained by killing Commander O'Neil, located in the Heart of Aeonia. Just head to the location of where the Grace point is on the map, and you'll find him. Defeat him, and you'll get the needle as a drop.

Return to Gowry. He'll tell you the secret of how to unlock the barrier in Sellia and that he'll need some time to repair the needle. Fast travel to another location, or quit the game and reload and speak to him again to get the repaired needle.

Get through the Sellia barrier by lighting the beacons in the city. Once through the doorway, keep heading up the hill, avoiding enemies and two giant cannonballs, and you'll eventually reach the Church of the Plague, with Millicent inside.

Speak to Millicent and give her the needle to use on herself. Exhaust her dialogue, then fast travel or quit and reload again, and she'll give you the Prosthesis-Wearer-Heirloom. Go back and speak to Gowry, and he'll sell you some sorceries to use.

Where to find Millicent in Elden Ring

Where to find Millicent after she leaves the Church of the Plague

Millicent goes to Gowry's Shack first after leaving the Church of the Plague, but only after you have spoken to him again for him to sell you sorceries. Reload the area, and Gowry should have disappeared with Millicent in his place. Exhaust her dialogue again, and she'll tell you that she's going on a journey.

The next location you can find Millicent at is just to the northeast of the Erdtree-Gazing Hill Grace site in the Atlus Plateau region.

To get to this area, you need to have found both parts of the Dectus Medallion to activate the Grand Lift of Dectus or found a cave that takes you up top.

Speak to her, and she'll say she wishes she could fight by your side. But alas, she cannot. To progress further, you need to find the Valkyrie's Prosthesis, found in The Shaded Castle, to the north of Atlus Plateau.

Once acquired, go back to Millicent, give her the Valkyrie's Prosthesis, and it'll progress the quest after exhausting her dialogue. Reloading, she'll disappear to a new location.

Where to find Millicent after she leaves the Erdtree-Gazing Hill Grace

From here, you must defeat the boss of Dominula, Windmill Village, located to the east of The Shaded Castle. Most of the enemies here are dancing, but they will turn hostile if you attack the dogs. Head up the hill to the top to encounter the boss.

After it's been defeated, fast travel to the Grace point there, and Millicent is just to the south. Speak to her and exhaust her dialogue before reloading to make her disappear.

Where to find Millicent after she leaves Dominula, Windmill Village

After Dominula, Windmill Village, Millicent travels to the Ancient Snow Valley Ruins Grace site in the Mountaintop of the Giants region. Speak to her once again to exhaust her dialogue, then reload.

Where to find Millicent after she leaves the Ancient Snow Valley Ruins

Next, Millicent travels to the Prayer Room Grace site in Elphael, Brace of the Haligtree. This area is a secret place that can only be accessed by finding the Secret Medallion to use on the Grand Lift Rold. This area is extremely difficult, so make sure you're high enough level or can dodge attacks perfectly when running through the area.

Once you have found the Prayer Room Grace, Millicent will be in that room, but she will not progress forward until you kill a mini-boss near the Drainage Channel Grace further on in the area. The Drainage Channel is the following Grace site, after the Prayer Room.

At the Drainage Channel Grace Site, take the head through the western door, climb the ladder, then go up the slope to the right, where the white lizard enemy is. Kill it, then at the top, turn left. Go over the log to the south, and jump over to the arena.

Walk into the center of the room, and the mini-boss will spawn. Defeat the mini-boss, then fast travel to the Drainage Channel Grace. Head back over to the mini-boss arena, then as you land on it, go right up the small slope to see the golden summoning sign. You'll be summoned by Millicent to help her defeat four challenging NPC enemies. And in doing so, you'll earn the Rotten Winged Sword Insignia.

Reset the area again by fast traveling to the Drainage Channel Grace site and returning to the mini-boss area once again. You should see Millicent straight ahead of you, dying. Exhaust her dialogue, then reset the area again to find her corpse with the Unalloyed Gold Needle on her.

Now, you need to defeat the boss Malenia, one of the most challenging bosses in the game. After the boss is dead, rest at the Grace site in that room, and a Scarlet Rot Plant will have spawned in the center of the room. Interact with it to have the Unalloyed Gold Needle turn into Miquella's Needle. And this is the end of Millicent's questline.
