Ulquiorra has a chance of being dropped each time you complete the Hollow Dimension Act 6 map on Nightmare difficulty in Story mode. At the end of the round, there's a very, very small chance the unit will appear in your rewards.

How to evolve Ulquiorra in Anime Last Stand (ALS)

At the time of this article's publication, Ulquiorra cannot be evolved. This will likely change in the future, though, and this unit will join the ranks of other evolvable units like Sukuna or Gojo as more updates are released.
