Enemy upscaling is an option listed under gameplay options that will increase the level of lower-level enemies to match Geralt's level. Enemies that are higher level than Geralt will be unaffected by this change. The upscaling feature also affects the level of loot dropped for Geralt from these enemies, which will also match Geralt's level. 

Enemy upscaling can be enabled in the start menu; select Options, Gameplay, and scroll down to enemy upscaling. The option can be toggled on and off anytime; however, you must reload the area before enemies' levels are adjusted.

Should you enable Enemy Upscaling in Witcher 3?

The short answer is yes. There are several benefits to enabling enemy upscaling. The level of loot that drops for Geralt will always be useful. Geralt will receive additional experience from defeating enemies, allowing him to level up faster. 

The downside to enemy upscaling is it can make the game significantly harder, especially on the death march difficulty, as every enemy encounter will be a challenge for Geralt.
