One such case occurs after recruiting Astarion. A cutscene at the campsite will occur then where he will attempt to drink your blood, and you will have a chance to stop him or let him feed. If you want to know what happens with each choice, this guide will help you make an informed decision.

What Happens if you let Astarion Suck Your Blood in BG3?

If you choose to let Astarion suck your blood in Baldur's Gate 3, you will instantly gain a single instance of approval with him and have a much easier time romancing him later on. So you should let him feed on you if you plan to pursue a romantic venture with him.

Aside from your nocturnal comrade's approval, you will also gain the vampire bite mechanic, which grants an additional action point in combat that has 100% chance to hit, giving Astarion +1 to attack rolls and ability checks. It also grants a chance to restore a little bit of health.

What Happens if you Don't Stop Astarion from Drinking Your Blood in BG3?

If you don't initially stop Astarion from drinking your blood in BG3, you will have two more opportunities while he is feeding on you. Each time you will have to do a dice check, and if you fail both times, he will kill the main character, letting you take control of another party member. This event doesn't give you a game over, because you can still revive yourself using a Scroll of Revivify (bringing you back with a single health point).

Can you Become a Vampire if you let Astarion Bite you in Baldur's Gate 3?

If you let Astarion bite you, you still won't turn into a vampire yourself in BG3. Since he is a vampire spawn, he doesn't have to power to turn someone into a vampire spawn by feeding on them, as such a power is reserved only for true vampires. He will offer the same explanation the following morning in the camp.
