You can get the Lamenter's Mask from the Lamenter's Gaol dungeon, northwest of Charo's Hidden Grave Region in the DLC. Specifically, you need to defeat the Lamenter, which is the boss of this dungeon to unlock this item. You'll have to traverse from the Jagged Peak to the Grand Altar of the Dragon Communion to reach Charo's Hidden Grave. From there, travel northwest and you'll find the Lamenter's Gaol.

How to defeat the Lamenter in Elden Ring Shadow of the Erdtree

The Lamenter is a relatively easy boss you can take down with ease. It has a couple of jumping attacks, an attack with its head, and a special attack that spawns clones of itself to attack you with Black Flame (these can be taken down with one hit). Whether you're a magic or melee build you shouldn't have a difficult time with this boss. The most noteworthy thing is it hits very hard, so make sure to dodge-roll away from its attack. After you've defeated it, you'll be awarded the Lamenter's Mask.

What does the Lamenter's Mask do in Elden Ring Shadow of the Erdtree?

The Lamenter's Mask turns you into a Lamenter while wearing it without any sort of armor. It also gives a small boost to your damage negation. This form also has a special ability where equipping the mask again will cause your head to swell and deal damage in the direction you're facing. However, it's not that effective with enemies who have high poise. The only way to remove the Lamenter form is by dying.
