Grand Theft Auto: San Andreas is the first game in the series to add RPG elements like physique. Players get the ability to work out after getting a call from Sweet.

Gyms are unlocked by receiving a phone call from Sweet after completing Drive-Thru, which introduces the player to gyms. Gyms are open 24/7. A player who loses 40% fat or gains 20% stamina or muscle in a single day must wait 12 hours before exercising again.

In San Andreas, there are multiple gyms, each located in the state's three cities and easily identifiable by the dumbbell icons on the map. The player will see their first one in Ganton Gym, Ganton, Los Santos, close by Grove Street.

Gyms all have four pieces of equipment:

TreadmillPerforming this exercise increases stamina and burns fat. There are 10-speed levels on the treadmill (the higher the level, the faster the player's stats will change). Higher levels require greater stamina. The power gauge must be full to keep CJ on the treadmill. If it empties, he falls off.
Bench pressExercises like this improve stamina and burn fat. 10 speed levels are available on the stationary bike (the higher the level, the faster the player's stats will change). The more stamina the player has, the easier the levels become. To keep the power gauge full, the player must cycle constantly. If it empties, they have to start over.
Stationary bicyclePerforming this exercise increases Muscle and burns Fat. Weights vary from 40 lbs (20 kg) to 320 lbs (145 kg). There is now a weird "press (the lift button or X on PS4) when highlighted area is hit," but pressing the lift button as quickly as possible works just fine.
DumbbellsMuscle is increased, and fat is burned during this exercise. The dumbells weigh between 20 lbs (10 kg) and 110 lbs (50 kg). The screen displays a power gauge and a rep count indicator at the start of a workout. The player must fill the power gauge before completing a rep.

If you don't keep CJ well fed, he will lose muscle and fat. Losing Muscle will make you weaker in combat, something you do not want to happen. Eating too much junk food and not working out will also cause CJ to lose muscle.
