What's new in the Baldur's Gate 3 epilogue ending?
The new ending takes place six months after the final battle, and it provides some much-needed closure in relation to companions and significant characters. In the new epilogue, Tav is gathering his group again at the High Hall Camp. There, he has a conversation with each one of them.
Now, for the best part, the topics of conversation and the behavior of the characters (or even their presence) largely depend on previous decisions you made through the game. This primarily works for personal quests that each character has.

New character epilogues in Baldur's Gate 3
Depending on your decisions in the game, endings for some characters may vary in a drastic way. If you let Astarion ascend in Cazador's mansion, his dialogue will be very different. Also, depending how you resolved Act 2's Nightsong, and her Act 3, Shadowheart will show various levels of sympathy. Even the absent characters will send letters you can read and close their character arcs.
The fan-favorite Karlach is one of the characters whose ending varied from bad to bittersweet. The new patch opened the possibility for a much better ending for her. If you help her become a mind flayer, she will appear in the ending. The same goes if she visits Avernus with Wyll or your character.
